Well maybe not the case here, it certainly can happen. It wasn't until my wife
Was in her last year at BYU that she learned her ACT score that she had been admitted with was not correct. She was working in a job where she had access to all student records and when she looked herself up one day she saw that her first ACT score which was low had been recorded correctly, but her second score which was significantly higher had been entered incorrectly so that her low score was the best score BYU had.

Fortunately it didn't end up biting her in the butt, but someone somewhere screwed up.
