I have never seen a person get suspended for a first time offense no matter
the offense. I didn't see every incident obviously. But lying about an incident and then getting caught in the lie is considered a second offense.

Also, there are no hard and fast rules about 1 time, or 2 times for anything. All incidents are reviewed specifically for the circumstances and the people involved. And I have seen a few where somebody didn't get suspended after 2+ offenses.
